Letter of Recommendation Process
Colleges and scholarships may require a letter of recommendation. Some may provide an electronic option. It is the student's responsibility to confirm the method in which letters should be submitted.
Click on the links below to obtain a "Letter of Recommendation Form". This is a form that students should complete when requesting a letter of recommendation from a teacher, mentor, boss, etc. Teachers at FHS will request this from a student when approached to write a letter.
Seniors should also be prepared to provide ACT, GPA and Class rank information along with school preferences.
This will help the author write a complete letter about the student.
Finally, provide approximately two weeks notice to the person that you are requesting the letter from. It is inappropriate to give short notice. Remember that manners go a long way when asking for a recommendation and a thank you note and update on your plans are always in good taste!

Please use this form if you prefer to use Google and plan on sharing it with the person whom you are requesting a letter from. MAKE A COPY OF THIS FORM and then complete and share when ready with the person who is writing your letter.
